Initial Visit
Expect to spend about an hour at your initial consultation. During this time, your provider will go over with you the questionnaire form you filled out prior to your visit to get a better understanding about you and your medical history. All labs and sono/mammogram reports you bring with you will be reviewed, as well as any medications/supplements you already are prescribed. A personalized treatment plan will often involve further lab testing.
How to Prepare
-
Questionnaire form is required to be filled out prior to your initial visit and sent to your provider
-
You should bring the recent blood test results and sono/mammogram reports
-
A list of current medications
-
Please bring the bottles of all vitamins/nutritional supplements so your provider can evaluate all the ingredients they contain
Lab Testing
At the time of your first consultation our provider may order additional blood tests. You will be provided with the lab requisition form. You should get your blood drawn at the lab of your choice.
Not all insurances cover all blood tests. We highly encourage you to check with your insurance prior to assuming your insurance will cover it. You may negotiate cash price with the lab prior to your test.
Payment
Functional and hormonal medicine services are not covered by your insurance plan. The Inspire Health is not a participating provider in Medicare or Medicaid. Payment is due at the time of service.
Cancellation Policy
In the event you are unable to keep your appointment, we request a 48-hour notice so that we can accommodate other patients waiting for appointments.
Follow Up
Three months after commencing the Hormone Replacement Therapy Program, you will be required to follow up with additional labs. This will determine that the dosages are where they need to be. If we are not satisfied with your levels, the regimen will be modified in order to adhere to your individualized needs.
Our provider will schedule appointments to continually monitor your hormone levels and to review the alleviation of your symptoms.
Follow up visits can be scheduled in person, by Facetime, or telephone.
